The FERPA Release of Information form is required in order to allow IU High School to release information about a student to:
- any other institution or individual other than the student's parent (for students under 18 years of age) or
- anyone including the student's parent (for students 18 years of age and older)
The form must be completed by the student (if 18 years of age or older) or by both the student and a parent (if under 18), and returned to IU High school before any FERPA-protected information can be released.
