The FERPA Release of Information form is required in order to allow IU High School to release information about a student to:
- any other institution or individual other than the student's parent (for students under 18 years of age) or
- anyone including the student's parent (for students 18 years of age and older)
If the student is 18 years of age or older, the student should fill out this student form.
If the student is under 18, the student and parent will need to fill out, respectively, this student form and this parent form.
